Your new company
An organisation providing enhanced community recovery services for people with complex mental health needs have the purpose or achieving independent or less supported living. The goal is early or timely transition to a community based, high support service. The core client group is psychosis and dual diagnosis.
Your new role
You will be working as an Assessment Lead Practitioner aiming to improve the rate at which people move into the service. You must ensure; effective management of the referral pipeline, quality and accurate assessments and fee costings, completion of assessments within 21 days of referral, liaison with area operational managers and a coordinated approach between assessment process and operations.
What you'll need to succeed
You will need a relevant professional qualification in health/social care e.g DIPSW, Social Work degree, Dip OT, RMN or equivalent.
