Alzheimer's Society is the UK's leading dementia charity. We provide information and support, improve care, fund research, and create lasting change for people affected by dementia. 

Position: Dementia Support Worker x2 
Location: Pontypool, Gwent (Covering Blaenau Gwent or Monmouthshire) 
Contract type: Full time, Permanent 
Hours: 35 per week 
Salary: £20,345 actual per annum

About the role: 

We are looking for two exceptional individuals to join our Dementia Support team. 

If applying for Ref VAC15755 you will provide support and guidance to people with dementia and their carers across the Blaenau Gwent/Caerphilly areas, helping to maintain their independence, improving their sense of well-being, and putting them more in control of their lives. 

If you are applying for Ref VAC15754 you will provide support and guidance to people with dementia and their carers across Monmouthshire and Torfaen. (Please specify on the application form which role you are applying for through the VAC number) 

The service is unique to each service user as it is based upon their personal circumstances and support needs. 

Within this role it is essential that you build relationships with a wide and diverse range of local contacts including, but not limited to, GPs, mental health services, carers support services and other community based organisations. 

As a Dementia Support Worker, you will coordinate, organise and facilitate the engaging and safe running of our Dementia Cafés aimed to inform, educate, and support those affected by dementia. The role will involve effective service user involvement, co-ordination and role management of service related volunteers. 

About you: 

We are looking for an enthusiastic individual who possess NVQ level 3 (or equivalent) with an understanding of dementia and the needs of those affected by dementia. An ability to assess the service user needs along with an understanding of the importance of confidentiality is essential. 

To be successful in this role, you will have excellent communication skills. As this role requires regular use of IT packages, therefore a good standard of IT would be desirable. 

You will be responsible for your own caseload therefore the ability to manage your own time effectively is essential. Your caseload will be varied so we are looking for someone with the right mix of enthusiasm, professionalism and flexibility who is able and willing to work in a variety of settings.
