The Role

We are actively recruiting a Case Manager who specialises in working with children and young people to join our team. We are looking to recruit a Case Manager on a part-time, permanent basis. There is flexibility with the hours worked to suit the Case Manager whilst also being able to meet the client’s needs.

We work with clients across East Anglia and into London. Therefore, the Case Manager will be expected to travel to visit their allocated clients in these geographical areas.

Full induction, training and supervision will be provided. Initially, during your induction period, you will be based at our offices in Suffolk. Thereafter, you will be based from home. Ongoing, continuing professional development will be offered.

Our Case Managers are provided with business support from our central team.

We offer Case Management advice, support and services for parents and professionals working with children, young people and adults; in particular those with brain injuries. The Case Manager

The Case Manager must be a registered professional with the Health and Care Professionals Council, Social Work England or Nursing and Midwifery Council. We would expect you will have extensive experience and a professional background as a Registered Nurse, Occupational Therapist, Physiotherapist or a Speech & Language Therapist who specialise in paediatrics or a Social Worker who specialises in children and young people.

The Case Manager will be expected to support clients who have had catastrophic and life changing injuries such as traumatic brain injury.

The Case Manager will be familiar working with clients in a community based environment. They will be competent in care planning and measuring outcomes for clients based upon the client’s identified needs. They will be required to develop positive working relationships with the client, their family and other involved professionals.
