Senior Care Assistant (Live in or Live Out Care, Rapid Response)

The Senior Care Assistant (Rapid Response Team) is a new role to support our clients transition from hospital to home.


  • Responsible for assisting the Care Service Manager and other clinical staff to transition (discharge) clients from hospital, care home or other services to Active Assistance
  • Enabling continuity of service and providing stability to care packages when not facilitating a client discharge
  • Mentoring new and inexperienced Personal Assistants and acting as a role model for safe and effective practice
  • Responsible for delivering the Clients care needs and assisting with activities of daily living


  • Enabling continuity of services often at short notice
  • Assisting with personal care, including skin care, bathing, shaving, oral care, toileting (including management of bladder & bowels), dressing, hair care
  • Enabling mobility, including moving & handling transfers, pushing wheelchair, checking of specialist equipment e.g. wheelchair seat cushion, driving of client’s vehicle, as directed (if applicable)
  • Dietetics, including assistance with menu planning, preparing, serving and storing of foods
  • Assist client to fulfil activities of daily living e.g. working, leisure, and education
  • Assist with medications as prescribed by medical team
  • Dealing with correspondence and financial matters as requested
  • Day to day household tasks e.g. changing beds, laundry, ironing, shopping
  • Adhering to company policies and procedures at all times including respect for client confidentiality and discretion.
  • Reporting & recording as per company procedure, including any changes to your Client’s care needs, observations and training records
  • Commitment for identifying and fulfilling ongoing personal development within Active Assistance
  • Observation and mentoring Personal Assistants as required
  • In the absence of client requirements, you will be expected to undertake various roles. Examples could be, standby, supervisions, administration or any other projects as determined by your Line Manager
  • Under the direction of the CSM:
    • Work between the Clinical team and Care Standards Officers to assist with ongoing assessment of care need for the client
    • Provide ongoing training support, observation and documentation of Personal Assistants working with the client; this will include assisting with care planning, observation and supervision



  • Two years proven care experience
  • Able to work flexibly, the role will require an ability to move around the UK to start up new packages of care
  • On occasions you may need to work unsociable hours as directed by Client need
  • Able to work on a live-in basis (living at the Clients home), or live in the location of the client (accommodation provided) for 3 months or more while initiating and stabilising a client package of care
  • Awareness of maintaining professional boundaries at all times
  • Independent, flexible, self-motivated and organised; with an ability to work as part of a team
  • Maintain a high level of personal presentation
  • Excellent written and verbal communication skills
  • Successful completion of our in-house induction training and Clinical Skills
  • Excellent time keeping
  • Willingness to study towards your Level 3 Diploma in Health and Social Care (if unqualified)


  • Level 3 Diploma in Health and Social Care / NVQ level 3 / BTEC level 3 (equivalent qualification is after 2010 only) / CACHE Level 3 Diploma in Childcare / NNEB


  • Level 5 Diploma in Health and Social Care / Clinical Qualification that is not registerable in the UK e.g. Registered Nurse, Physiotherapist, Psychologist or Occupational Therapist
  • Full driving licence
  • Access to own car

Hours of work and Benefits:

  • Salary - £23,000 - £25,000 dependent on qualifications (overtime will also be available based on hours worked)
  • Contracted average of 160 hours a month however due to the nature of the work overtime will be required and this can either can paid as time off in lieu or paid in overtime
  • Annual Leave - 22 days plus Bank Holidays. You will be expected to work some Bank Holidays. This will need to be agreed and three months’ notice will be required
  • Auto enrolment into NEST pension scheme
  • Shift times will vary as directed by the client need
    • This will either be on a live-in basis (living with the Client) or
    • live out (living in accommodation provided by Active Assistance which will be shift cover (day and night shifts) and some weekends
  • You may be required in some circumstances to live at the client premises for an agreed period of time
  • Whilst on a live-in assignment accommodation will be provided, whilst on a live out assignment accommodation will be provided by Active Assistance locally to the Client
  • Whilst not on assignment accommodation will be provided or an allowance will be paid for accommodation
  • Whilst on agreed annual leave accommodation will not be provided
  • Food provided by the Client or an allowance (whilst working)
  • Access to funded training
  • Access to workplace wellness scheme
  • Full training will be given including on occasions working in a hospital or similar setting to gain the skills required to work with our complex clients
