Senior Clinical Pharmacist

Flexible hours of working

North Lincolnshire East and West Primary Care Networks are recruiting a Senior Clinical Pharmacist within each network to develop medicines optimisation services within the network, travelling between practices.

You will arrange face-to-face medication reviews of patients with polypharmacy and those with multiple co-morbidities; working directly with patients to assess and treat conditions, as well as promote self-care.

You will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing.

You will have a post-graduate diploma or higher degree in clinical pharmacy and will be an independent prescriber. Ongoing development will be supported within the national programme. You must be fluent in written and spoken English, have experience of leadership, influencing others and conducting clinical medication reviews. Your contract of employment will be held by Safecare Network Ltd.
