Practice Based Clinical Pharmacist Job Description for Cambridge City 4 Primary Care Network

We are looking for a Clinical Pharmacist (equivalent toband 7/8a) to join our primary care network to help revolutionise how we care for patients. We are excited about adding pharmacists to our team and expanding their role in our organisation. The Cambridge City 4 PCN consists of 6 practices in a close geographical location on the south-east edge of the city. The total population covered is 55,000patients, with a wide range of health needs.

The appointed pharmacist will be based at Cornford House Surgery, 364 Cherry Hinton Road, Cambridge, CB1 8BA and will work alongside a Band 8a pharmacist who is already in post. They will have remote access to all practices. They will spend time in all practices and become embedded in their teams. The appointee will be an independent prescriber or will receive training for this goal. They will have a varied role including patient facing services, audit, care-home medication, improving safety and quality as well as prescription queries and updating discharge summaries. They will be a strong team player, with excellent communication skills and enjoy working closely with colleagues and patients. We aim for them to be an essential part of the team at Cornford House, but will also have links with other practice based pharmacists in the Cambridge area. They will have a GP mentor who will meet and support them on a regular basis. They will have experience of working with GP practices and local Community Pharmacies.
