OPAT Clinical Co-ordinator

Band 5

Are you passionate about providing high quality care for patients? Interested in being instrumental in the implementation of a new service? Could you be a part of our new OPAT team? We have a vacancy for a highly motivated individual who is passionate about providing and developing services that actively respond to the needs of patients who require intravenous antibiotic therapy.

We work closely with our colleagues in the community, social care, and of course across the Acute Trust to provide comprehensive assessment, seamless transfer of care, appropriate follow-up and rapid co-ordinated discharge where and when appropriate.

You should have experience of coordinating care of patients across their whole pathway, working with multidisciplinary teams across acute and community settings, ensuring swift response to the changing clinical needs of patients. You will be highly organised with attention to detail to ensure all required information is collated and reported, both to support clinical care for patients, and assurance on the delivery of this new service. Experience of working with colleagues and advising patients remotely as well as in face to face situations.

To join our friendly and dynamic team for either role - you will need to be an enthusiastic, energetic, proactive practitioner committed to your professional development and that of others.

Croydon staff receive on-going support and development in their role, including hands on guidance from practice development nurses, senior members of the team and in-house and external courses. Training at the Trust also supports nursing revalidation, helping nurses to build their portfolio of CPD evidence and competency documents.
