Our client, a Local Authority based within West Yorkshire, is seeking an interim Senior Social Worker for its Hospital Discharge to Assess Team.

Rate of pay is £30 + per hour

The role will involve the following:
  • Working collaboratively around Delayed Transfers of Care (DToC) and Length of Stay (LOS)
  • Overseeing hospital discharges on a discharge to assess pathway
  • Identifying, challenging and developing solutions to any possible safeguarding risks for vulnerable residents
  • Representing the Local Authority at a range of meetings, proceedings and reviews as required

Requirements of the role:
  • Knowledge of discharge to assess pathways
  • Qualified Social Worker
  • Social Work England registered
  • Enhanced DBS â€" this can be provided at no cost by REED
