Leaving Care Personal Advisor needed for Devon County Council, £11.24 ltd an hour

Start Date- ASAP
Contract End date- 3 months initially
Hours per week - 30 hours (Tuesday - Friday)
Pay rate - £11.24 ltd an hour
Team - Care Leavers
Location - Tiverton

Description
Manage a caseload of yp 16-25. Maintain monthly contact with yp, maintain reviews of Pathway Plans, Promote relationships with carers and families. To routinely undertake interviews, observations and information gathering and to analyse, summarise and evaluate this information into an assessment of the Care Leavers needs. Ensure yp are aware of their entitlements and support yp to manage their money, liaise with schools, colleges and universities to increase educational attainments. Work in partnership with other agencies to ensure yp has suitable accommodation, education/training/work, ensuring good mental, physical health.
