Lynx Services are currently recruiting for a senior coordinator on behalf of our client

Key Responsibilities

1. To support the screening process for referrals to the Integrated Coordination Hub received into the service ensuring timely and accurate allocation of next steps and Key Worker allocation;

2. To support the patient caseload management by using evidence based / client centred principles to support the assessment, planning and implementation of discharge planning for patients with discharge planning needs;

3. To maintain highly accurate and detailed records;

4. To develop skills and knowledge through participation in structured development programmes and competency framework;

5. To consolidate and build on existing skills and knowledge specific to the needs of patients with complex discharge plans;

6. To support a comprehensive assessment process for patients including those with diverse or complex presentations/multi pathologies; using effective organisational skills to track and support, facilitate safe and timely discharges;

7. To take delegated responsibility from the Discharge Hub Leads for tracking patients with particular emphasis on discharge planning and be responsible for providing accurate and timely information, data and clear plans for those patients;

8. Support the formulation of accurate and comprehensive discharge plans by working closely with the Assessment Leads;

9. Liaise and work alongside other Hub colleagues, agencies and professionals regarding outcome from assessments, e.g. Therapists, Social Workers;

10. Understand and have a knowledge of the legal framework for patients who lack capacity to consent to discharge plans and proposals;

11. Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress discharge plans. This will include patients who may have difficulties in understanding or communicating, for example, patients may be dysphasic, depressed, deaf, blind or who may be unable to understand the information provided;

12. Evaluate patient progress, reassess and where necessary highlight and/or escalate any deviation to discharge plans;

13. To highlight risk within patient caseload to Hub Discharge Leads, submitting incident risk forms as appropriate;

14. Work within Trust and Local Authority guidelines in relation to discharge and to have a good working knowledge of national and local standards;

15. To be responsible for maintaining accurate and comprehensive patient records in relation to discharge planning;

16. To maintain record keeping systems according to service standards, departmental and Trust guidelines;

17. Represent the Integrated Coordination Hub and/or individual patients at the multi-disciplinary team meetings, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate clear discharge plans as part of the treatment programme. This will include discussion of patient care, patient progress and involvement in discharge planning;

18. Will need to undertake weekend work on a rota basis without supervision
