We have an exciting opportunity for a Personal Advisor to support young people in North Manchester.

  • The Personal Adviser will be responsible for supporting young care leaver between the ages 16-21 with their independence to include accommodation, health, finances, promoting relationships with friend and families as well as supporting them with their education, employment and training activities.
  • Building positive relationships with the young people, carers, birth family, partner agencies and professionals supporting the young person.
  • Supporting 28 young people that will range from ages 16-21.
  • Completing 8 weekly in touches, completing pathway needs assessments, pathway plans, pathway reviews.
  • Using Liquid Logic system and inputting information on the young person's file.
  • Attend team meetings, Leaving Care Service Meetings as well as visit the young people in the community.
  • Undertake duty work in the Leaving Care Office.
  • Working remotely from home and will come into the office for duty work, attending meetings etc. Will need to abide by the COVID 19 visiting home requirements and be able to work from home.
  • Dealing with complex cases involving young people issues such as NEET, drug, alcohol issues, mental health, domestic abuse, UASC, physical health problems, homelessness, transition to new accommodation, DWP benefits, supporting them with independence skills such as budgeting, cooking, building relationships, promoting health and well being.
  • Work closely with social worker for the young people aged 16-18 along with placement providers, education and health colleagues.
  • Attend LAC, PEP and other meetings.
  • Travelling around Manchester but also to other LA areas to see young people

  • Experience of supporting young people
  • Case management experience
  • Pathway Planning experience
