Your new company

You will be joining an established organisation which places the service user at the heart of everything they do. This organisation is focused on supporting vulnerable people in the community. This organisation primarily supports service users are whose lives have been held back by a range of social deprivation, poverty and social exclusion.

Your new role

Please note we have roles for this position also in the London areas too:
  • Deliver street outreach shifts aimed at reducing rough sleeping in the Reading area
  • Assess the un-met needs of those who are rough sleeping with mental health or dual diagnosis difficulties
  • Establish contact with rough sleepers and beggars; working as part of a small team to rapidly assist clients through street outreach shifts at a variety of times and locations in the Reading area
  • Divert people who are new to the area when appropriate and encourage and assist people to return to their home communities
  • Identify, target and engage entrenched rough sleepers via street outreach, including those who are unwilling to engage with services
  • Participate when required in street counts
  • Work proactively to ensure that clients have access to ongoing healthcare to access mental health and substance misuse services where required
  • Identify individuals who are not ready to engage with support services. Encourage them to consider the options available and deliver interventions aimed at supporting increased motivation
  • Alongside service users, work as part of a team to look at both housing needs, and mental health / dual diagnosis needs to devise and carry out agreed action plan
  • Work in conjunction with statutory and non-statutory service providers on care planning and reviewing, attending external meetings as required
  • Encourage and empower service users to make best use of appropriate statutory and community resources
  • Establish and develop links between services that ensure service users with a dual diagnosis access the full range of care and treatment services to meet their needs
  • To work in line with the requirements of funder's


What you'll need to succeed
  • Must have Outreach experience
  • Must have worked with homelessness and clients with substance misuse issues
  • Willing to work shifts on rota basis
  • Must be available within weeks notice
  • Ideally on DBS Update Service


If you are not on the Update Service Hays will process new Enhanced DBS for you free of charge.
