We have a requirement for a business administration member of staff to support one of our public sector clients who work jointly across Pembrokeshire County Council and the Hywel Dda Health Board.

The team support the timely discharge of patients from care and also support individuals to remain in their homes, avoiding the need to attend hospital.

The role is for a Care Co-ordinator brief summary of the duties below:

- To take referrals from professionals and refer to other specialist services using agreed pathways.

- Some knowledge of health/social care would be good (but not essential)

- Required to have strong communication and interpersonal skills

- Able to extract and record information therefore confident using bespoke IT systems

- IT skills

- Able to work on initiative and be flexible.

- Be comfortable working at a fast pace.

- May include some weekend working / working outside standard 9am-5pm hous

- The role will be based either in County Hall, Haverfordwest with some home working possible .
