About the job:

As a Brokerage Officer for Cambridgeshire County Council through Opus Cambridgeshire, you will focus on the Discharge to Assess (D2A) service across three acute sites (Addenbrookes, Hinchingbrooke and Peterborough City Hospital) to coordinate care provision for those requiring care home placement or Domiciliary Care.

As a Brokerage Officer to the council you will:

- Working within Brokerage D2A function. Sourcing care for referrals across all acute and community hospital settings, providing support to the wider Brokerage team.
- Working effectively with Supporting Professional Activities (SPA), Nurses and Transfer of Care Team (TOCT) Social Workers across adult social care and their managers.
- Leading to ensure the effective use of available capacity, responding to service requests for the provision of care across Cambridgeshire and Peterborough.
- Modelling best practice in care placement and seeking to continuously develop the business process to achieve a timely and customer focused approach to meeting the needs of people who require care to live as independently as possible in their own homes and in residential/nursing homes.
- Use of data management tools and systems to understand and share information on the supply and demand for Home and Community Support and placements in residential/nursing homes. Maintain accurate data to obtain and provide accurate statistical, financial, provider and management information.
- Maintain and develop effective partnerships with Care Provider’s on a day to day basis to sustain confidence, trust and responsiveness. The Post Holder will liaise with care providers by telephone and email on a daily basis and will be required to provide care Provider’s with composite information on the care packages/placements required.
- Ensure that accurate information/case notes are created in the case management system (Mosaic) to concisely describe the action that has be taken to secure care provision for service users, and also on the D2A spreadsheets
- Negotiate with providers to maximise use of available capacity and achieve best value for the Local Authority.
- To demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.

Qualifications/Experience:


The successful candidate will have excellent communication/inter-personal skills and be empathetic to the needs of vulnerable adults. This is a fast moving role and you will need to be well organised and be able to prioritise workloads. Previous experience within a customer focussed environment is required with the ability to work with highly confidential and sensitive data.
