Are you a qualified Social Worker with experience in working with Hospital Discharge Teams? Do you enjoy helping with ongoing needs through conducting assessments and reviews? We have a new exciting opportunity to join a tight knit team based in Llandough in Cardiff offering an attractive hourly rate of GBP17.50.

Responsibilities that you will have during this role

* Working within a 'short term team' which means that you would work with specific service users conducting initial assessments based on the current needs and then making referrals onto other teams on a longer term basis.

* Conducting assessments and reviews of individual cases.

* Helping the transition as a service user moves out of a hospital setting and into residential care or into community care.

*Making applications to panel for funding for any ongoing care the person will require.

Requirements :

You will need to

* have a Social Work degree
* be registered on the Care Council for Wales
* be able to drive
* have a full enhanced DBS
