We are recruiting for a Clinical Care Manager for our Acquired Brain Injury services, you will be expected to provide a high quality case management service to clients who have an acquired brain injury in order to promote independence and ensure the best quality of life.

Avenues East is a charity dedicated to providing high-quality, personalised support to adults with an acquired brain injury (ABI) and to people with learning disabilities. Our reputation is built upon our experience, our values and our commitment to support people to live independently and achieve their goals.

You will be part of a small case management team and will have a very manageable portfolio of supported living and outreach clients. The role includes working with a support team to ensure the delivery of a person centred rehabilitative support plan for each client in your portfolio. This includes:

* Setting goals to assist clients in rebuilding their skills and confidence.
* Assisting with cognitive rehabilitation and implementing cognitive compensation and communication strategies
* Supporting client families during the period of adjustment including providing advice and training on the brain injury rehabilitation programme.

Active support means that instead of doing things for people or to people, you are enabling people to engage in all the activities of everyday life, no matter how disabled they are. This will increase inclusion, independence, choice and control.
What knowledge and experience should you have?

* Knowledge and experience of supporting people with an acquired brain injury
* A qualification in either Occupational Therapy or Speech Therapy
* A good working knowledge of Mental Capacity Act and how it would impact on your work
* Knowledge of current CQC Framework

What skills and abilities will you need?

* Good communicator
* Negotiation/conflict resolution
* Ability to manage oneself
* Ability to apply a common sense approach to suit the situation
* A full UK driving license and access to your own vehicle
