Main Purpose of the Job

To provide a high standard of reablement care and non invasive health treatment for clients referred into the Community Independence Service.
To work in a multi agency environment with clients to instil a reablement service focus. To provide care which focuses on the needs of the individual, promotes independence and choice and maximises quality of life.
To avoid admissions to more institutional forms of care.
To provide a dignified and respectful service that encourages and motivates disabled, frail and older people to live active lives of their choosing.


• Physically fit to carry out the tasks of the job.
• Strong degree of common sense. Whilst there will be training and reablement goal plans to follow, the role is dealing with human beings where common sense and confidence to raise issues and ask for guidance to address matters arising is essential.
• Ability to understand the process of assessment, planning, implementation and evaluation of care and follow a reablement goal plan.
• Excellent communication skills.
o Ability to converse clearly, patiently and sensitively with clients who are from a wide range of cultures and backgrounds and involve them in making decisions about their care.
o To be able to build and maintain effective working relationships with staff and service users.
o Ability to accurately interpret and record client information on paper or IT systems.
• Competence with IT skills.
• Experience gained of caring for vulnerable people, or the ability to demonstrate an understanding of the needs of vulnerable people and the potential to learn how to meet their care needs.
• A working knowledge of Health and Safety issues and the ability to gain a working knowledge of legislation.
• Self motivated, takes responsibility for own actions
