Salary: circa GBP34,000 - GBP39,000
Hours: Full-time, 37.5 hours week - Part-time and job-share considered
Location: Leatherhead, Guildford or Working, Surrey - with some local travel

Our client is a leading private provider of physical and mental health services in the UK, working in partnership with the NHS.

A key role has arisen for a Nurse Specialist (also known as Utilisation Management Clinician). The Nurse Specialist will play a vital role in ensuring that high quality, clinically necessary care is being provided in the most appropriate and cost-effective therapeutic setting.

The Nurse Specialist will be responsible for collaboratively reviewing, with care teams, the flow into (front door) and out (back door) of community-based services and assistant with proactive and comprehensive discharge/aftercare planning to maintain case flow across the system and making the most efficient yet safe use of child health resources.

Key Responsibilities & Accountabilities:

*Undertaking peer-to-peer telephone or in-person care plan reviews with partner care providers for service users who have fallen outside of expected care pathway timeframes
*Determining appropriate level of care (i.e. care setting) for service users based on clinical necessity level of care criteria
*Managing an allocated Utilisation Management caseload
*Interacting with provider clinicians to discuss clinical necessity questions, alternative treatment options and concerns regarding specific cases
*Providing specialist clinical advice, guidance and consultation to other professionals
*Keeping accurate clinical records of work undertaken with appropriate documentation and communication of same within and external to the service in accordance with good practice recommendations and policy guidelines
*Providing information to service users, families and providers regarding community treatment and care resource options
*Providing telephone triage and crisis intervention to callers and, when necessary, assists with ensuring service users/family members have access to local services and resources
*Assisting in collection of outcomes information, annual analyses and other reporting and initiatives
*Assisting with quality assurance initiatives
