I am looking for a skilled, compassionate and care focussed care coordinator to join a person centred provider of bespoke care and support packages for adults with learning disabilities.

Based within a friendly Stratford Upon Avon office, you will join a provider who specialises in the provision of unique care and support within supported living, respite and domiciliary care settings.

You will join an organisation with a great reputation and has a brilliant track record with CQC, as all their services are currently carry a "good" rating.

As care coordinator, you will be responsible for managing and coordinating an enhanced team of support staff and ensuring that planned and emergency cover is provided across all services. You will work directly with the service managers and effectively manage cover for unallocated shifts, resourcing issues and team allocation.

You will utilise the Staff Plan system to manage staff planning and coordination across the Warwickshire and Worcestershire services, as well as ensuring all unallocated shifts are covered and that overtime is monitored.

You will provide concise and accurate reports, ensure all training is up to date and that the care teams hours are filled fully.

As with all care coordinator roles, you must be willing to work the "on call" rota and undertake the duties of a social support worker as required.

A competent social care professional, you will be proactive, person centred and have great CQC knowledge. You will have excellent communication skills and be committed and driven to make this new service work.

Qualified to NVQ 3 in health and social care, you will have excellent computer literacy and experience of electronic rostering is essential to be considered for this job role.

You will hold a full UK driving licence and have access to your own vehicle.
