We are recruiting a Practice Manager within the Transition Team, working with disabled young people between the ages of 16 - 25. It would be an advantage if you have experience of working with Children and Adults with disabilities.
This is a unique opportunity to be part of a newly created, more integrated team with scope to shape and build a stronger service for Disabled Children and Young People.

As Practice Manager for the Transitions you will be working in a progressive, committed and learning environment where staff are supported and valued. You will play a key role in developing penalisation and integrated working as we take forward this new way of working your leadership skills, energy and enthusiasm will be vital in our drive towards making sure that every disabled young person's needs are met.

The successful candidates will be expected to work across the borough according to the policy of mobile and flexible working.

The successful candidates will be responsible for carrying out and coordinating
assessments under the Care Act and the Children Act including carer's assessments as well as reviewing and monitoring the progress of young people as they progress through adulthood. The team will also play an active part in contributing to EHC plans under the Children and Family's Act to enable our young people to achieve good outcomes in:
