Key responsibilities

Overall the Director will work with the Programme Director for Population Health to provide leadership, support and direction in developing and implementing a population health approach across the UCLPartners footprint. This will include supporting the further development of population health IT tools, including providing

Coordinating the development of bids for funding and helping to develop and evaluate how best to scale-up the use of evidence-based practice in new models of care for patient benefit.

Working with partners in primary, community, social care and secondary care along with patients and the public and local authorities, the role will provide UCLPartners' leadership in responding to Sustainability & Transformation Partnership strategies and transformation plans, leading towards the successful transition to accountable care across partners.

As a member of the UCLPartners Director Group, the post-holder will carry a corporate responsibility for representing and supporting the wider vision and purpose of UCLPartners, and for managing a sustainable medium-sized company
