Title: Care Coordinator
Location: Islington Branch

For 20 years ANA Nursing has been dedicated to providing care to those who wish to remain in their own homes long after they are able to manage all the daily chores of doing so themselves.

We are currently looking for a Care Coordinator to assist our Care Manager at our Islington Branch.

You will help in the day to day operation of the service and be the right hand of the manager. You will be responsible for:-

* Producing the carers rota
* Providing both written and verbal instructions to the carers
* Be aware of the new regulations and requirements that have recently come into effect - Care Act 2014, Care Certificate etc.
* To assess service users needs
* To ensure that all service users have up to date personal care plans that are accessible to the whole team and provide the standard of care expected of the company. The care plans are to be reviewed regularly.
* To supervise and manage a team of carers within a geographically defined area.
* To carry out home visits to service users in line with Company policy and procedure.
* To undertake care work in an emergency situation. To be flexible, enthusiastic and have a "can-do" approach about all aspects of the job.
* To cover the out of hours telephone on rotation with the team.
* Assist in the day to day operation of the home care business
* Preparing staff rosters, to ensure the proper continuity of on-going delivery of care. Re-allocating shifts as necessary, to cover staff absences.
* Accepting, allocating and processing referrals.
* Providing written reports on complaints, accidents and incidents

We are looking for the following skills and attributes as a starting point:
- Experience in care provision at a level that reflects the responsibility of the post
- Good literacy and numeracy skills
- Computer literate
- Excellent communication skills are a must
- Driving would be an advantage
