Experts agree that people with dementia and long term mental illness are, where possible, best looked after in their own homes.

The Mental Health of Older Adult Service provides a social work and care management service to people aged over the age of 65 years of age with dementia or mental health problems living in the borough of Lewisham. Co-located within the SLAM (South London and Maudsley) Mental Health for Older Adults Clinical Academic Group, the team works closely with consultants, CPN's and other multi-agency professionals in a service that offers a range of community based mental health services within Lewisham with the aim of ensuring that everyone receives a level of treatment and care that is appropriate and effectively co-ordinated to help maintain health and to help people to live as independently as possible.

The role

As a Senior Social Worker within this service you will carry a case load of complex cases, be responsible for assessing needs and formulating support plans that promote the independence and wellbeing of those with the most complex needs to enable them to live in the community. Using a range of skills and resources you will work with service users and their families to decide on the best way that support can be given when people are discharged from hospital, either back into the community or where no longer considered viable, into supported accommodation or placements.

Managing risk and ensuring that safeguarding concerns are appropriately addressed will be essential components of your role. As a qualified AMPH you will be part of the borough wide AMPH rota and ideally, as a qualified BIA (Best Interest Assessor) you will maintain your practice by participating in the borough's BIA rota.

You will have strong links and working relationships with Lewisham's Neighbourhood Teams and other Adult Social care teams, and be part of a wider Directorate that is committed to delivering high quality, creative and outcome focussed services to the residents of the borough.

What we're looking for

  • A HCPC registered social worker with evidence of 2 years post qualification experience
  • Proven background in Mental Health Work
  • Approved Mental Health Practitioner Qualified
  • Ideally you will be BIA (Best Interest Assessor) trained, or be prepared to train.
  • Excellent track record in complex frontline practice and in working in a multi-disciplinary team
  • Confidence and professionalism
  • Analytical skills and a reflective approach
  • Ability to make decisions and manage crisis
  • Detailed knowledge of relevant legislation, policy and best practice
  • A personality that makes you an engaging, informative source of guidance and support
  • Self-motivated with an ambition to develop
  • A desire to contribute your ideas and share good ways of working
