==JOB ROLE==

** Responsible for the assessment, planning, implementation and evaluation of individual care plans.

** Participate in a range of assessments including risk and self-harm.

** To supervise a designated number of Health Care Assistants and students.

** To assist in standard setting and evaluating quality outcomes.

** Expected to take charge of the ward/Unit in the absence of the person with continuing responsibility, under the direction of senior staff.

** To work as a Primary / Named / Key Worker advising and supporting junior colleagues.

**To maintain good standards of communication, both oral and written, in accordance with NMC guidance.

** Responsible for reporting to the Ward Manager any circumstances that may affect the delivery of care and/or efficient running of the ward.

**To provide supervision to other staff, qualified and unqualified.

** To ensure the security of the unit and all who use it and that policies and procedures are in place and followed.

** Maintain effective relationships with service users, their relatives, friends, staff and 3rd parties.

==KEY RESPONSIBILITIES==

* Work collaboratively as a Primary / Named Nurse / Key Worker within the multidisciplinary team, including liaising with associated professionals and other agencies.

* Ensure that patient centered care plans are developed, implemented, evaluated and recorded.

* Ensure effective communication with service users, relatives, visitors and staff. Facilitate ward/unit activities.

* Ensure risk management issues are assessed, identified and care planned for, any issues of concern communicated within the multidisciplinary team.

* Ensure effective communication with patients, carers, visitors and staff. Make effective use of evidence based practice in day-to-day clinical practice.

* Implement and ensure effective use of the Learning Disability Act where necessary.

* Ensure safe standards apply to the administration of medication.

* Ensure adherence to policies and procedures at all times. Assist with the development and/or revision of policies and procedures as necessary.

* Supervise and monitor the use of supplies and equipment ensuring supplies are adequate and used appropriately.

* To be an effective role model to all staff within the working environment. Participate in teaching programs with all staff including students. Have an awareness of own development and training needs. Maintain a learning environment. Assist in the induction of new staff.

* Participate in clinical supervision with a personal supervisor. Supervision/Mentorship of other staff. E.g. unqualified nurses and nursing students
