-To fulfill the requirements of the Children Act 1989 transition to adulthood for care leavers and Children (Leaving Care) Act 2000.

-Act as PA for young people leaving care to provide support, advice and assistance to enable young people to maximise education, training, work and accommodation opportunities for young people between ages 18 and 21 (or up to 24 if in higher education).

-To hold an allocated caseload be responsible for the provision of direct services to care leavers. This will involve carrying out assessments of need, Pathway Plans for Eligible, Relevant and Former relevant young people and providing practical and supportive services.

-To provide individual personal support and advocacy to young people.

-To carry out specific tasks and duties in relation to complex cases where the case responsibility is held by an allocated social worker.

-To undertake these duties within agreed departmental service objectives, polices and procedures and to promote the Council's Equal Opportunities Policy
Principal Accountabilities and Responsibilities:

-Engage with young people to identify and address their needs, complete Pathway Plans, offering information, advice, guidance and support on learning and career options and personal development opportunities, with a view to raising the aspirations of each young person.

-To assist the young person with their transition into permanent housing and furnishing the property.

-Work with and support education and training organisations and employers.

-Work with a network of voluntary, statutory and community agencies.

-Manage information effectively to facilitate the process of meeting the needs of young people.

-Provide structured and individual support to young people using the Pathway Planning process to create an individual plan with agreed personal development targets.

-The ability to budget management with care leavers.

-The ability to undertake Mental Health Capacity Assessments, Ordinary Residency Assessments, Age Assessments and Human Rights Assessment is essential.

-Actively engage young people in decision making about their future choices and enable their development into effective adults by providing impartial support, advice and guidance.

-Manage a caseload of young people, regularly review their progress and assess the level of support required through regular review of pathway Plans.

-To provide young people with practical advice in relation to budgeting, preparing for independence, safe care/sexual health advice and household management and to provide basic rights welfare advice etc.

-Develop a thorough understanding of available local provision and make appropriate referrals.

-To respond appropriately to young people in times of crisis.

-To maintain awareness of current legislative requirements as relating to care leavers and to participate in relevant training.

-Carry out duties with due regard to the Council's Customer Care, Equal Opportunities, Information Governance, Data Protection and Health and Safety policies and procedures.
