If so an exciting opportunity has arisen for a qualified and experienced health or social care Professional with expertise in NHS continuing healthcare, to take a lead role in developing integrated practices in Dorset.

You will provide leadership to our teams across Dorset who are carrying out the NHS continuing healthcare application processes.

You will use your expertise in NHS continuing healthcare and the lawful limits of LA provision, to work with health colleagues in developing, transforming and implementing new models of working for those individuals with continuing health care needs in Dorset.

This position will require you to have experience of managing complex social or health care services and leading others through change. You will have the ability to influence and deliver strategies and joint policies to effect positive change and to build effective relationships with health colleagues and Senior Managers to develop integrated practices.

You will require excellent communication skills to develop relationships and work closely with health partners to negotiate and resolve continuing healthcare eligibility matters.

The role will require you to plan, implement and evaluate focused pieces of work around emerging themes and priorities to support the work of the Adult social care Directorate.

As well as being responsible for managing and developing the performance of your team, your own continuing professional development is important and Dorset County Council will work flexibly with you to enable access to development opportunities in support of your career aspirations.

You must have a degree or equivalent in relevant Social Care or Health profession, be registered with the HCPC and be able to fulfil the travel requirements of the post.
