Salary Range: GBP25,951 - GBP37,306 pa
Hours: 37 per week
Contract: Permanent

Hertfordshire Adult Care Services are looking to recruit an experienced Social Worker to support the assessment and care co-ordination of Hertfordshire Residents being discharged from Out of County Hospitals. This will include Luton and Dunstable and Stoke Mandeville Hospital. The team are based primarily in Apsley (Hemel Hempstead) and Luton and Dunstable. You would be expected to be flexible and be able to travel to all Out of County Hospitals.

You will need to have completed your ASYE and have some relevant experience. You will need to be HCPC registered and have knowledge and experience of relevant legislation, demonstrate knowledge and experience of safeguarding and have knowledge of multi-disciplinary working. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.

The team promotes personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support.

Supporting people with the right care and information at the right time can help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.

To improve outcomes for service users our Hospitals Teams support a 7 day Hospital Discharge service. At present staff are working on consecutive Saturdays and Sundays once every eight weeks and are receiving days off during the weeks before and after covering weekends. This can be reviewed.
