The Post

You will be responsible for managing a range of cases for children and young people throughout their transition into adulthood.

The responsibilities will include assessment of needs, developing support plans for children and young people, undertaking reviews, completing carer's and adult needs assessments, attending LAC review meetings, carrying out child protection investigations, attending relevant meetings with partner agencies and attending court hearings.

The Person

Do you have the relevant professional qualification in Health or Social Care?

Previous knowledge of the needs of children and young people with learning or physical disabilities and of relevant legislation?

Can you test and determine eligibility criteria for services (children and adults)?

Are you able to work positively as a member of the multi-agency team on complex and multi-disciplinary cases and work positively with children and young people with their families to ensure positive outcomes are achieved as a result of Transitions Team involvement?
