Salary Range: GBP25,951 - GBP37,306 pa
Hours: 37 per week

Older Peoples Services (Herts Valleys area based at Apsley, Hemel Hempstead) are looking for experienced Social Workers who are enthusiastic, dynamic and resilient to join Three Rivers Older People Extended Involvement Team. You will need to be HCPC registered and have knowledge and experience of relevant legislation in relation to social care, safeguarding and mental capacity. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.
Older Peoples Services promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enable older people to direct their own support. We need practitioners that can engage with older people by providing, promoting and enabling inclusive community options as well as self-directed purchased services. You will be a resilient, imaginative and reflective practitioner who has the ability to work in a busy environment.
This post requires you to visit service users in the community to undertake assessments and reviews, so a full UK driving licence is essential as is the ability to respond to situations quickly and flexibly.
You will have proven success in showing good communication skills in building relationships with service users; families; carers and professional colleagues in a multidisciplinary environment.
