The newly formed ITT at Stepping Hill Hospital is looking for an enthusiastic social worker to fill a temporary gap in staffing to provide an assessment and care planning service to a range of acute hospital patients. In the hospital there are some exciting new initiatives which include the development of a Trusted Assessor and a Transfer to Assess model of service delivery as well as SAFER model of ward based MDT process. Under the umbrella of Stockport Together the integration of professional health and social care groups, provider organisations and the third sector is developing to support safe and timely transfer of patients to the setting most appropriate to meet their immediate needs. Staff have all come together under one roof in order to enhance communication and reduce duplication of effort in order to achieve a more joined up service for the benefit of patient and services alike.

The successful candidate will work alongside colleagues from the Foundation Trust - discharge coordinators, AUK/Targeted Prevention Alliance, Pennine Care Mental Health Trust and the CCGs CHC nurse assessors as well as other aligned providers of service.

You will be a qualified and registered social worker with recent experience of working in an acute hospital where you have undertaken the range of social work responsibilities including assessment under the provisions of the Mental Capacity Act and the Care Act. You will be able to work effectively and efficiently whilst delivering a person centred approach to assessment and care planning.

You will develop close working relationships with MDT colleagues and you will provide fast track service for people who are both, medically optimised and safe for transfer, into services in the community. Early identification of patients needing assessment, the effective liaison with MDT members and, thorough needs assessment/skilful care planning, will need to be supported by good organisational and administrative skills.

You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
