Sal ary Range: GBP25,951 - GBP37,306 pa (pro rata for part-time)
Hours: 37 per week
Contract: 1 Year Fixed Term / Secondment Opportunity
An exciting opportunity has arisen to join our busy, fast paced, team. We are looking to recruit an experienced Social Worker who has experience of working with individuals who have had an inpatient Mental Health Service and their families to support safe and timely discharge from acute units.
The successful applicants will able to evidence sound knowledge of safeguarding practice, working with people with dementia, continuing health care process, personalisation, Individual Budgets and multi-disciplinary working. You will keep the individual and any future potential individual at the centre of all activity and address user needs within the legislative framework.
The successful applicant will be responsible for assessing and arranging discharge from a non- acute hospital setting, covering a large and diverse area. This role would welcome applications from candidates who can bring energy, creative care planning, citizen focus and an understanding of delayed transfer of care.
