This is an exciting opportunity for adult social care professionals to get involved in a brand new 24 bed rehabilitation focused service in partnership with Camden and Islington NHS Foundation Trust.
This service will provide an integrated approach to rehabilitation and recovery and act as a hub for excellence and innovation. Care and support staff will work alongside in-house NHS clinicians, providing a fully holistic multidisciplinary approach to help customers regain their independent living skills and the confidence to apply those skills.
This will be a flagship service, one of the first of its kind to provide this type of integrated approach, which builds on the success of One Housing's integrated supported housing model. It will provide the best care for customers by creating the right physical space, the right support staff, and proactive specialist clinical and therapeutic input.
In addition to providing a high quality service to customers, the service will also excel in its work with community partners, ensuring external resources feed into the care and support for our customers.
We are looking to recruit Band 5's (Full & Part time) for this service, with the right applicants being able to work collaboratively to empower people using recovery and person-centred approaches, that are creative and flexible, to motivate people with complex mental health and physical health needs, and promote hope, enthusiasm and optimism.
