We are looking for an energised and passionate Project Worker like you (known internally by us as a Support Co-ordinator – Level 3) to join our service which provides support to customers within the Breckland District Council and West Norfolk. Our new service is focused on reducing homeless by supporting customers who are either homeless or at risk of becoming homeless.
We are focused on engaging with people who may find it difficult to interact with other services as a result of their support needs which may include mental health, substance dependency or physical health conditions.
You'll work proactively and creatively to identify customers who would benefit from the service, deliver a street outreach service where needed, set up and deliver group and drop-in activities with customers and to develop excellent relationships with the local councils, housing providers, social care, health, information advice and guidance services and local community resources.
Our support model is focused on colleagues having a lower caseload but delivering an intensive service to make a real change with positive outcomes for some of our most vulnerable customers.


As Lead Support Worker you will manage a caseload of clients, working closely with external agencies to deliver person centred support. We know that one size doesn’t fit all, so our clients lead their own support every step of the way. Together you will really make a difference, empowering our clients to achieve their goals and aspirations as outlined in their support plans and develop the independent living skills they need to successfully move on with a positive future.

You will help shape our services, undertake risk assessments, support planning, goal setting and regular reviews. You’ll also guide and allocate tasks to our Support Workers too. It’s really important to us that you have time to what you are really passionate about; making a real difference to our clients lives. So to free up your time, you’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office as you can complete support plans on the go. You’ll also be paid between clients visit and for your mileage too.

A bit about you

We want people like you, who strongly believe that providing support is more than just a job and can make every client feel valued. To flourish in the role, you will be a like-minded dedicated person who can demonstrate compassion, empathy and creativity in your approach and use your unique skills in enabling clients to reach their full potential. With previous experience of working with vulnerable people in a support or care environment, you will have a great understanding of professional boundaries, safeguarding, risk assessment, confidentiality and be experienced in case management. It’s also important that you can work in holistic way building partnerships with external agencies and that you have strong IT skills too.

The core delivery for this service will be Monday - Friday however evening and weekend work will be needed in order to ensure that we are available for our customers when needed. We need you to have a driving licence, car and car insurance business cover as travelling will be required although we envisage the majority of our customers will be based in market towns in the borough.

If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which we’ll pay for.
