Working between a community and hospital setting, you will carry out initial customer assessments with regards to patients preparing and facilitating discharge from the acute hospital. This will entail working with medical professionals to facilitate the effective discharge and smooth transition to the patient’s home. You will provide outcome focussed support planning and subsequent customer reviews in the community and will need to gain and maintain a comprehensive knowledge of available local community resources and pro-actively liaise with our integrated team of health and social care workers, as well as other external health and social care professionals, to maximise the support available to the individual.

We are looking for exceptional people who share our values and our commitment to helping and caring for individuals; you will be expected to hold or working towards level 3 QCF in the care industry and be an Approved Assessor. Experience gained in a hospital setting would be desirable.
